Qualifications:

For nearly thirty years, the Ridgewood Bushwick Senior Citizens Council, Inc. has been a beacon of hope, providing crucial social services to Ridgewood, Bushwick and the surrounding communities of Brooklyn and Queens. Since its inception, Ridgewood Bushwick has assessed and monitored the changing needs of the community and has been able to provide a comprehensive system of services to some of the most needy youth, seniors, and families in New York State. As an action oriented agency, Ridgewood Bushwick's goals have been to continually seek strategies to uplift and stabilize the community and empower local residents. Our mission is to provide programs that offer a holistic approach in aiding families to achieve their goals. The RBSCC HomeBase Program is a Department of Homeless Services (DHS) initiative designed to reduce homelessness in the Bushwick, Williamsburg, and Brownsville communities. Our primary objectives are to identify and outreach to persons who are either homeless or at risk of homelessness, stabilize those households, address the underlying causes of each housing crisis, and foster long term health and stability. HomeBase offers its clients comprehensive case management services, housing relocation and assistance, referrals, and financial assistance through a DHS subsidy fund.
This position assists Brownsville residents to avoid entering shelters by providing housing assistance and holistic case management services. HomeBase case managers provide intake, assessment, engagement, counseling, management and referral services.
Responsibilities:
1. Overall case management services to include: completing a full case assessment and the development of the housing and services plan for each client and family member.
2. Work with each client to develop a financial assistance package if applicable.
3. Close follow up of service plan goals and actions to ensure client outcomes.
4. Refer individuals/family members to needed service providers, i.e. substance abuse treatment, domestic violence services, employment training and placement programs.
5. Assist clients in finding housing.
6. Provide adequate follow-up via home visits/telephone contacts.
7. Ensure access to public benefits and entitlements.
8. Document and maintain records in a timely manner for each client.
9. Develop an operational knowledge of entitlement programs, city wide charitable organizations and local service providers.
10. Participate in training, weekly supervision and unit meetings as required by the Case Management Supervisor.
Case Managers report directly to their Senior Case Manager and Case Management Supervisor.
