Job Description: Responsible for providing nutritional health education to LCHC patients. Provide individual nutrition assessments/ consultations for LCHC clients referred by providers and other appropriate health center staff. Focus will be on diabetes nutrition, cardiovascular related nutrition and weight management. Participate in pediatric and adult weight management clinics.
