As a Bilingual Case Planner you will provide comprehensive case management and support to children and families by:
Monitoring childrens health, school performance and living situation.
Referring birth parents to drug rehabilitative services and therapeutic services or other community resources as needed.
Scheduling and conducting office visits, home visits and sibling visits.
Provide casework counseling to foster parents, adoptive parents and birth parents.
Preparing case documentation (i.e. progress notes, psycho socials, FASPs, and Reports of Investigation).
Attending court adoption hearings, TPRs, trials and extension of Placement.
Attending clinical and case management meetings
Meeting all program contractual requirements and adhere to agency standards
Completing additional job related tasks as needed
