BSW Required to fill this position and MSW is preferred.2-3 years of Case management experience.
POSITION OBJECTIVE:Under the direction of Director of Program Development will be responsible for ensuring the provision of person-centered, consumer responsive services to the consumers and families served at CAHS, Inc. Works to oversee and implant the Services as described in CAHS Client Treatment Plan and OMIT principles are in place for all individuals served.
MAJOR DUTIES/RESPONSIBILITIES:
To have full and complete knowledge of all government (Federal State, City, Court, etc.) regulations relating to OMH and CR-SRO Housing.
Providing case management and clinical services to an identified case load of 25 consumers to include maintenance case records/notes, consumers service care plans and all other pertinent clinical documentation.
Maintain complete and accurate resident records per NYS OMH and other regulatory agency guidelines.
Direct consumer services, i.e. case management, group facilitation life tutors, planning and provisions for recreation/respite, budgeting, etc.
Provide crisis intervention and prevention for consumers, if and as needed.
Conduct consumer/intakes as necessary, per agency and guidelines.
Works with families as needed, including making home visits to facilitate the effective supports and the consumers’ treatment plans.
Preparation, coordination and participation in in-service training and group counseling sessions for consumers, families and staff as needed.
Provide individual counseling for caseload.
Participate in CRO, SRO meetings and community meetings, as needed.
Monitor the administration of medications and service treatment plans for caseload.
Accompany consumers to and from activities and appointments, i.e. social, recreational, medical, dental and other appointments when necessary.
Attendance/participation in citywide committees, representing CAHS, Inc. in a professional, prompt and punctual manner.
Act as a liaison between CAHS, Inc. and other community-based organizations/services in securing necessary services/benefits for caseload, as needed.
Notify Director of Program Development/Director of Residential Services in the event of critical and/or emergency incident.
Any additional responsibilities as defined by Chief Executive Office, Director of Program Development/Director of Residential Services.

CRITICAL RESULTS/IMPACTS:
The Case Manager should have a comprehensive knowledge of community resources and a thorough understanding of the consumer’s personal preferences and needs.
