MUST HAVE VALID NYS DRIVERS' LICENSE to be considered for this position.MUST be willing to carry an agency cell phone.
SCOPE OF POSITION: The Medical Coordinator is directly accountable to the Residence Manager and in his/her absence to the Associate Executive Director. The Medical Coordinator will ensure that he/she carries out their job in accordance with agency policies and all funding source regulations/guidelines (i.e. NYS OMRDD, US Dept. of HUD, NYS Labor Dept. and Federal Regulations, etc.) to safeguard consumer’s rights and confidentiality as it pertains to consumer and employee files.
MAJOR DUTIES AND RESPONSIBILITIES:
Under the supervision of the Residence Manager responsibilities include, but are not limited to the following:
Will also function as Direct Care Counselor as needed.
Provide all residents with transportation to and from scheduled hospital and clinic visits. Coordinates medical appointments for Lodovick Avenue ICF.
Post weekly appointment schedule, file consumers’ paperwork and communicate with the agency medical staff.
Maintenance of all agency vehicles, including registration, inspection, tune-up, interior and exterior cleaning, etc., monthly or when necessary.
Maintain a valid NY State Driver’s License.
Attend and participate in all shift meetings and full staff meetings.
Notify Residence Manager in the event of an untoward incident, to which you report.
Appropriate dress for this position is in accordance with the agency’s dress code.
Any additional responsibilities as deemed necessary by the Residence Manager, Coordinator of Residential Services, Associate Executive Director, Chief Operating Officer and Chief Executive Officer.
Assist residents in with getting up in the mornings, dressing and personal hygiene as apart of preparation for daily activities.
Must stay in compliance with OMRDD regulations in order to dispense medications to consumers.
Ensure that all residents are properly groomed and exhibit appropriate dress and hygiene at all times.
Assist residents and staff in the direction of resident services including ADL training, provisions for recreation, group activities including assisting residents in the preparation of breakfast, lunch, dinner and snacks when necessary.
Conduct fire evacuation drills as instructed by Supervisor/Residence Manager/CRS
Maintain and complete accurate resident records
Read and initial communication log on a weekly basis.
