Specific Education, Certification and Skill Requirements (if applicable): CM1: HS Diploma and 2 years experience. For CM2: Bachelor’s Degree, or HS Diploma and 4 years relevant experience. Note: for every 30 college credits earned, 1 year of experience may be reduced from the requirement for applicants with HS diplomas. Job Description: The 350 Transitional Living Community (TLC) is nationally recognized model for helping mentally ill homeless women acquire housing. Program services include: transitional housing for 43 women, comprehensive case management, group treatment, on-site psychiatric and medical services, and housing placement. We are looking for both a Case Manager I & II. Resp: The Case Manager is responsible for providing a full range of direct services to clients with particular emphasis on mental health, health and substance abuse services. The position involves individual and group services as well as collateral contacts.
