Specific Education, Certification and Skill Requirements (if applicable): BA in human services/related and QMRP qualified. Must have 3+ years experience/training in field of human services/working with people with DD. Supervisory experience, proficiency in computer skills, and valid driver's license and personal vehicle also required.
Job Description: Transition Program Manager manages and oversees day-to-day operations of a home that serves 6 participants with developmental disabilities; is responsible for the growth and development of the participants in the program and markets the Transitions Program to internal and external communities. Develops transitions curriculum and QMRP paperwrok and coordinates staffings. Ensures human rights and safety assurances. Develops positive behavior support plans and coordinates outings
