Specific Education, Certification and Skill Requirements (if applicable):
Have at minimum a Bachelor's degree in Social Work, psychology, nursing, community health or a related filed; must have at minimum 2 years experience in community health or similar setting; experience working in communities on Chicago's West Side preferred; experience working with families and/or young children preferred; bilingual in English/Spanish preferred. Ability to work well with a diverse population.

Job Description:
This position is located at the West Side Future YMCA. Under the direct supervision of the Program Director, the Bilingual Case Manager will provide direct services to families with a pregnant woman and/or infant who are eligible for the Healthy Start Management Program. Services include intake and assessment, service planning, referrals and linkages for additional services, assistance with supportive services, counseling, advocacy, health, nutrition and parenting education, monitoring, and follow-up services. The Case Manager will document services provided and maintain customer files in accordance with agency guidelines and funder requirements. In addition, the Case Manager will maintain regular contact with referring agencies. Local travel is required.

Primary responsibilities: Complete client assessments and verify program eligibility. Develop along with client, an individual care plan for each client. Track and monitor client progress toward program goals and assist them with meeting the goals established in their respective care plan. Follow-up on outreach activities and maintain a caseload within guidelines established by the program director at the start of the program year. Maintain monthly contact with clients and conduct home visits at required intervals. Track, monitor & follow-up with client progress according to program standards. Make appropriate referrals to external resources as needed. Document services provided to clients per established guidelines in YMCA paper records and IDHS client information system. Participate in client staffing, community and program meetings. Organize, conduct and participate in community education workshops, in-service trainings, and related events.
